IPU Frailty Specialist

At Garden House Hospice Care, our team of passionate professionals bring specialist expertise to ensure the best support and care is available for people living with frailty and life limiting conditions today, tomorrow and in the future.

Join us as we transform our services, addressing imbalance and inequalities to reach more people living with frailty earlier on in their journey. This exciting career opportunity offers the time to care, time to talk, time to listen, time to grow and time to innovate along with continuation of NHS service and pension.

This role forms part of a new and innovative service provision led by the Hospice to support patients with recognised moderate to severe frailty and two or more of the following comorbidities: heart failure, renal failure and respiratory failure. We are looking for registered nurses with a minimum of 2 years’ experience of life limiting disease management to support with the delivery of our new frailty model. Are you the one we’re looking for?

You will lead on the development of an ambulatory model of care that would provide care closer to home for patients living with frailty, heart failure, renal failure, respiratory failure & cancer. This will involve working closely with the medical team and wider allied health professionals with the Hospice.

A message from Lisa Hunt, CEO at Garden House Hospice Care:

“Our whole philosophy is to enable our patients to live their best life, however long that might be. That means we need to be there for them every step of the way. Managing their symptoms, enabling them to enjoy their lives. Holding their hand at bumps in the road and supporting their families when they need it most.

“Our staff understand that, and they do it every day. They are the ones who best understand the patient and family’s needs. We, the senior management team at Garden House Hospice Care truly want to improve every day, because every person is different and everyone’s needs are different. We need to be adaptable, we need to be agile, but most importantly we need to listen. It’s our staff we listen to. Because they are the ones who understand the daily challenges and also know the answers.”

What our staff have to say:

In a recent staff engagement survey, 85% of staff rated Garden House Hospice Care as an excellent place to work, with 98% of staff saying they enjoyed working with the team.

“I have been with the organisation for 12 months, and in that time I can honestly say we are improving every day. We bring our teams together at every opportunity and we live our values.

“We care for patients and we care for each other. Everyone I come into contact with at the Hospice is dedicated to making a difference. The low patient to staff ratio really gives you the time to care and is a real privilege.”

You’ll have opportunities to grow and develop your skills as part of a committed and talented team with career progression, the opportunity to be involved in developing new initiatives, Learning & Development programmes and Continuous Professional Development. We reward our staff with autonomy in their role and a supportive culture that encourages healthy work life balance.

We recognise that our staffing team (including our volunteers) are our greatest asset. Recruiting, developing and retaining our staff is a high priority for us, amongst the benefits we offer to support this are:
