Enhanced Nursing in Care Homes

Our Frailty Specialist Care Home team work with care homes, liaising with residents, their families and carers around future care wishes.

The Enhanced Health in Care Homes (EHCH) service across East and North Hertfordshire is led by Stort Valley Healthcare Ltd as the lead provider, with Garden House Hospice Care supporting the delivery of the service as part of the multidisciplinary team. Our Frailty Specialist Care Home team, made up of nurses and occupational therapists who work closely with care homes to provide holistic assessments, create personalised care and support plans, and guide important conversations around advance care planning.


The approach empowers residents, families, and carers to understand conditions, recognise early symptoms, and make informed choices. By involving families and respecting personal wishes, the team help ensure that every person can live as well as possible - and when the time comes, experience a good death in the place they call home.


This service aims to tackle inequalities in our aging population, offering training and support to care home staff while supporting the Central East Integrated Care Board (ICB)’s ambition to reduce the number of A&E admissions.

How we can help

  • Providing holistic assessments of residents on admission to the care home, or where frailty is identified
  • Giving information to families who can then go into conversations with other healthcare professionals better informed about their relative's condition, options and needs
  • Using our knowledge to signpost to other appropriate services
  • Offering training and support for staff with advanced care planning and difficult discussions with residents and families
  • Signposting to specialist education

Referrals

Our Frailty Specialist Care Home team support contracted care homes in East and North Hertfordshire. Staff there can refer patients for reviews by our team by completing our professional referral form. This can be largely auto-completed through Ardens or SystmOne.
