IPU Frailty In Reach Nurse

This role forms part of a new and innovative service provision led by the Hospice to support patients with recognised moderate to severe frailty and two or more of the following comorbidities: heart failure, renal failure and respiratory failure. We are looking for registered nurses with a minimum of 2 years’ experience of life limiting disease management to support with the delivery of our new frailty model. The focus of this post is to ensure a 7 day in reach model of care with ENHT at Lister Hospital to support, assess and enable the identification of those patients with recognised moderate to severe frailty and two or more of the following comorbidities: heart failure, renal failure and respiratory failure who are eligible for transfer of care to GHHC.
