What will I learn?

• To understand what ‘NHS Continuing Healthcare’ means and how it relates to discharges from the in-patient unit
• To complete an NHS Continuing Healthcare Consent form, Checklist, and Fast track form
• To have an awareness of the Decision support tool
• To understand the NHS Continuing Healthcare process in different areas
