The hospice is pleased to have secured funding for two, one-year education posts to provide End of Life Care education in nursing homes in East & North Hertfordshire.
The roles were successfully filled by Caroline Smith, previously Practice Development Nurse and Jane Landon, previously Hospice at Home Sister at Garden House Hospice Care.
The new education posts will focus on delivering 2 Day On-site End of Life Workshops to Care Home and Residential Care Staff. This ABC programme is a free of charge end of life care education programme specifically offered to nursing and residential homes in East & North Hertfordshire. The proven programme will increase the confidence of nursing staff and their ability to support people who are at the end of their lives and prevent unnecessary hospital admissions at end of life.
A recent independent government commission report ‘Review of Choice in End of Life Care’ (which focuses the government commitments to end the variation of end of life care) highlights the work of East and North Hertfordshire Clinical Commissioning Group’s Better Care for Hertfordshire Care Home Residents Vanguard Programme – part of NHS England’s New Care Models programme.
The Vanguard focuses on improving the health and quality of life of all care home residents.
It brings together East and North Hertfordshire Clinical Commissioning Group, Hertfordshire County Council and the Hertfordshire Care Providers Association to promote good health by:
• supporting all homes with multi-disciplinary teams of health experts, including dieticians, geriatricians, pharmacists, mental health professionals, dedicated link GP practices for each home and therapists
• improving the training, expertise and confidence of care home staff
• providing a ‘rapid response’ team of clinicians to assist residents in failing health within 60 minutes of a call
• granting GPs mobile access to comprehensive information about each patient during their visits.
