In Reach services

Supporting people in our community with symptoms linked to long-term conditions, preventing hospital admissions.

Over the past two years, we have transformed and expanded our Hospice care to reach more people living with frailty and advancing illness. What began as a response to changing community needs by introducing our In Reach services (formerly known as the Frailty Service) has become one of the most important developments in our Hospice journey.


We know that hospice care is no longer about the final days of life. Increasingly, people are living for years with complex, life-limiting conditions that require compassionate, specialist support much earlier. Demand for palliative and end-of-life care is growing rapidly, and Garden House Hospice Care has evolved to meet that need.

Renaming the Frailty Service

Our experience has shown us that the term 'frailty' does not always resonate with patients and families and can fail to reflect the breadth of people who may benefit from hospice care. We now more often use the phrase 'advancing illness' - a more inclusive and compassionate description that encompasses all people living with a long-term condition.


By opening our doors wider, we can help people access support earlier, improving quality of life, reducing crisis situations, and ensuring more individuals and families receive the care, dignity and reassurance they deserve throughout the last phase of life, in the place they choose.


This work is shaping the future of hospice care: proactive, person-centred, and rooted in the belief that every person facing advancing illness should have access to expert care, comfort and support when and where they need it most.

Founders of the Frailty Service

When we first launched the service in January 2024, we launched a public campaign to raise the additional funds needed to help us achieve our plans. Any individual, company or trust who gifted over £5,000 in the first three months of the service became a 'Founder of the Frailty Service'. With special thanks to:
