Rehab and Wellbeing
We provide care and support to inpatients with any life-limiting condition. Our high staff-to-patient ratio ensures that patients get the dedicated support they need.
Our community team of nurses and healthcare assistants provide care for patients in the place they call home, 24 hours a day, seven days a week, 365 days a year.
When someone is diagnosed with a life-limiting illness, everybody in the family is affected. We will meet you wherever you are in your journey, pre- or post- bereavement.
Our Frailty Specialist Care Home team work with care homes, liaising with residents and their families and carers around future care wishes, putting Advanced Care Plans in place.
