Specialist palliative care for patients, their families and carers at the Hospice and in the community.
Launched at the beginning of 2024, our frailty service supports people with symptoms linked to their increasing frailty, preventing hospital admissions and long waits in A&E.

Our expert team will support you to maintain skills, confidence and optimise your quality of life, enabling you to live well with your diagnosis and creating a treatment plan.
Rehab and Wellbeing

You may be referred to our Inpatient Unit by your GP for reasons including short-term symptom relief or psychological support, or to provide care in the last days of life.
Our community team of nurses and healthcare assistants provide care for patients in the place they call home, 24 hours a day, seven days a week, 365 days a year.

When someone is diagnosed with a life-limiting illness, everybody in the family is affected. We will meet you wherever you are in your journey, pre- or post- bereavement.
Our Frailty Specialist Care Home team work with care homes, liaising with residents and their families and carers around future care wishes, putting Advanced Care Plans in place.
