Self referrals

You can self-refer to our Rehab & Wellbeing and Emotional & Psychological Support services.

Rehab & Wellbeing

Our team of physiotherapists and occupational therapists will support you to maintain your quality of life, enabling you to live well with a life-limiting illness and continue doing the things in life you enjoy. We support with symptom control issues, providing one-to-one and group rehabilitation in a friendly, relaxed environment.

Emotional & Psychological Support

When someone is diagnosed with a long-term condition, everybody in the family is affected. Patients known to the Hospice, their family and friends can self-refer for emotional, psychological and supportive care from our team. We will try to meet you wherever you are in your journey, whether pre or post bereavement.
